The plate

Printed, hand-written, or printed and reinforced with an NFC tag, the plate scales down to the bare minimum or up to whatever you're willing to fuss with. Defaults here are fine for most books.

I'm also sticking an NFC tag in next to the plate

Program the tag to open the same link the QR code does, that link is in the email you get after this step. The plate will then read "tap or scan" instead of just "scan".

Print my name on the plate too, if I gave one above

Your name already appears on the public journey page if you give one; this only decides whether it's also on the paper strangers hold.

One more language on the plate, if it'll help

Skip it if you don't know where the book is headed, English carries fine on its own. Printing all six just to be safe is what made the old plate so busy.

Coming home

Optional. Most books never come back, and a book that wanders forever is arguably the better outcome. But if you'd like a shot at it, you can ask.

Ask a reader to post it back after a while

Nothing goes public until you record where you left it.
